यानंतर कॅप्चा आणि मोबाईल नंबर टाका
फोनमध्ये Beneficiary Search पेज उघडेल.
त्यात पीएम-जे योजना निवडा आणि तुमचा राज्य, जिल्हा आणि आधार क्रमांक भरून लॉगिन करा.
यानंतर ज्या कुटुंबाचे आयुष्मान कार्ड बनले आहे त्यांची यादी दिसेल. मात्र ज्याचे कार्ड बनलेले नाही, त्यांच्या नावासमोर ऑथेंटिकेट असा पर्याय दिसेल
ऑथेंटिकेटवर टॅप करा, आधार क्रमांक टाका- ओटीपी टाका आणि फोटोवर क्लिक करा.
यानंतर, सदस्याचा फोन नंबर आणि नाते लिहा.
ई-केवायसी पूर्ण करा आणि फॉर्म सबमिट करा.
एका आठवड्यात व्हेरिफिकेशन होईल आणि त्यानंतर तुम्ही हे कार्ड अॅपवरून डाउनलोड करू शकता.
आयुष्मान कार्डसाठी लागणारी कागदपत्रे
आयुष्मान कार्ड बनवण्यासाठी आधार कार्ड, फोन नंबर, रेशन कार्ड आणि पासपोर्ट आकाराचा फोटो लागेल. तसेच कामगार कार्ड, ई-श्रम कार्ड किंवा सरकारी ओळखपत्राच्या मदतीने तुम्ही कार्डसाठी अर्ज करण्यास पात्र आहात की नाही याची माहितीही मिळेल.
